Nasal bleeding presents differently across age groups, with distinct common sites for young and elderly individuals. In young people, nasal bleeding predominantly occurs in the anterior part of the nasal cavity, whereas in the elderly, it is more frequently found in the posterior part of the nasal cavity. Specifically, about 90% of nasal bleeds in young individuals are located in the anterior nasal cavity, particularly in the anterior inferior part of the nasal septum, known as Kiesselbach's plexus. This area is a convergence zone for the head and face's blood vessels, which originate from the carotid artery system, including both the internal and external carotid arteries. These vessels eventually form a network in the anterior inferior part of the nasal septum. Due to the superficial nature of the mucosa in this vascular network, and the relatively exposed blood vessels, any external irritation can easily lead to rupture and bleeding. The bleeding can range from mild blood-streaked mucus to severe arterial bleeding, which can be pulsatile and cause significant anxiety in patients.

Nasal bleeding in the anterior part of the nose is particularly common in children and adolescents. This is related to the anatomical structure of the nasal cavity, where the anterior inferior part of the septum is a site of vascular convergence. Additionally, air entering through the anterior nares and reaching the lungs directly exposes this region to various airborne irritants, which can damage the local mucosa. Furthermore, children may also suffer from nasal bleeding due to habits such as nose-picking.

In contrast, elderly individuals experience nasal bleeding more frequently in the posterior part of the nasal cavity. This is due to age-related changes, such as arterial sclerosis, and the presence of a venous plexus in the posterior part of the lower nasal passage. These venous structures are sensitive to blood pressure fluctuations, making them more prone to rupture and causing posterior nasal bleeding. Consequently, nasal bleeding is more prevalent among the elderly due to these vascular changes.
